Form 3 is to be used ONLY by tap teachers, organizations, schools, institutions (or their official representatives), studio owners, tap program directors, etc. It should NOT by used by teachers or assistant-teachers. Studio owners who are/were also tap teachers should use this form but if you do not own a studio or direct a tap program you should NOT use it. Instead you should fill out Form 2.
Part B of this form is very important as it will greatly facilitate our understanding of and communications with those who are interested in being notified of upcoming opportunities to ORGANIZE or HOST events featuring NTE cast members (you received the scoop on anything that's planned for your area, with first pick on date and times of your choice within a given tour.) We highly recommend that properly qualified people get "in the loop" and "in the know" by filling out Part B. Please note that even if you do not, you will still be automatically informed by email of all open-to-the-public NTE events that are organized/scheduled in your area (even if someone else is organizing or sponsoring those events) just like all ITDN members who joined by using Forms 1 or 2. Again, there is NO obligation to book or host any class or program, you will just be given the OPPORTUNITY to do so. This service is also entirely FREE.
